GMCH :Achieves Surgical Milestone with Successful Repair of Giant Ventral Hernia in 77-Year-Old Woman

Udaipur: In a remarkable achievement in advanced gastrointestinal and abdominal wall surgery, the Department of General Surgery at Geetanjali Medical College & Hospital, Udaipur, successfully performed a highly complex repair of a giant ventral hernia measuring approximately 15 x 20 cm in a 77-year-old female patient from Raebareli, Uttar Pradesh.

The multidisciplinary surgical team comprised Dr. Sanjeev Agrawal, Head of the Department of General Surgery, Dr. Mohit Kumar Badgurjar, anesthesiologists Dr. Alka Chhabra and Dr. Seema Partani, along with Dr. Krishna and the dedicated operating theatre team including Hemant and Deepak Sen.

The elderly patient had been suffering for several years from a progressively enlarging abdominal swelling associated with chronic pain, restricted mobility, and significant impairment in daily activities. Given the size and complexity of the hernia, the case posed substantial surgical and perioperative challenges.

Following detailed clinical evaluation, Dr. Sanjeev Agrawal and his team formulated a carefully planned staged management strategy. To optimize surgical outcomes, an innovative preoperative technique known as Progressive Preoperative Pneumoperitoneum (PPP) was employed under the supervision of Dr. Mohit Kumar Badgurjar. A catheter was inserted into the abdominal cavity, and controlled air insufflation was carried out over a period of approximately one month. This gradual expansion of the abdominal cavity increased intra-abdominal volume and facilitated safe reintegration of the herniated viscera during definitive repair.

Subsequently, the surgical team performed a complex abdominal wall reconstruction using the advanced Posterior Component Separation Technique with Bilateral Transversus Abdominis Release (TAR). This highly specialized procedure is considered one of the most effective yet technically demanding approaches for the management of giant and complex ventral hernias.

The surgery was completed successfully without major complications. Postoperatively, the patient demonstrated excellent recovery, with significant improvement in mobility and quality of life. She has since resumed her routine daily activities and is leading a normal life.
